As a parent or legal guardian, I agree not to hold SPX Camps or SPX Camp Staff responsible for injuries sustained by my child while participating in camp activities. I understand that in the event of an emergency, every effort will be made to contact me and other designated contact persons listed below. If I cannot be reached, I hereby grant permission to SPX Camp Staff to secure proper medical treatment for my child. I carry medical insurance on my child and will provide SPX Camps with that information if necessary.
I understand that photos of my child may be used on the SPX school or camp websites, and/or in camp advertisements. My child's name will not be listed with the picture.
Medication and Allergy note: If your child requires an inhaler or other medication that is usually taken during the course of a normal day, or if an Epi-Pen may be needed as a just-in-case measure for your child, please bring it in with your child the morning of Camp. We will log your child’s medication when it is dropped off the morning of the Camp, it will be kept in a secure location, and the medication will be returned to you when you pick up your child at the end of the day. The correct paperwork must be on file in the Summer Camp office. More information may be found in question #21 on the Summer Camps FAQs page.
To complete your registration, please forward your non-refundable deposit ($40 per camp) to hold your camper's spot, or full camp payment via one of our payment options :

*PayPal and credit card payments, unlike most other types of payments, have a fee assessed by the credit card company. This fee is called "Interchange" and is calculated as a percentage of your payment amount. The percentage varies by credit card company and type of card but all credit card companies charge "Interchange". We charge you a "Convenience Fee" to recover the cost of the credit card "Interchange". Our intent is not to make a profit in this "Convenience Fee" it is simply to cover our cost of providing you the "Convenience" of using your credit card to make a payment.
